Local Access to Coordinated Healthcare (LATCH) from DukeWELL is a program managed by the Duke Population Health Management Office (PHMO). LATCH provides care coordination services to uninsured and underinsured Durham County residents. LATCH services include assistance with medication, transportation, access to primary care, mental health, affordable dental providers, care coordination of Project Access of Durham County (PADC) patients, and Disability Services.
